ALPHA MART — CONTRIBUTOR PACK

For anyone filming or photographing people for Alpha Mart.

Four pages. Read the first one. Print the other three and take them with
you — a photo of a signed form on your phone is fine, but the form has to
exist before you start.


01 FILMING BRIEF
   Everything you need before, during and after a shoot, on one page.
   Read this even if you read nothing else.

02 APPEARANCE RELEASE — ADULT
   One per person, 18 or over, who appears.

03 PARENT PERMISSION — UNDER 18
   MUST BE SIGNED BEFORE FILMING. Not after, not at the end of the day,
   not "we'll sort it out later". If a parent has not signed, the child is
   not filmed. There is no exception, including when the child is keen and
   the parent is standing nearby.

04 ALLERGY AND PARTICIPATION
   For anyone who will eat or drink on camera. Completed before anything
   is opened. Many Indonesian snacks contain peanuts.


THE SHORT VERSION

- Permission in writing, before you film. A message counts for adults.
  For a child it must be the parent, on the form.
- Ask about allergies in writing, in advance, of everyone eating.
- Anyone can withdraw at any time. We take it down first and talk after.
- Never film another customer's face, basket or payment without asking
  them on the day.
- Never film inside the shop without Alpha Mart present.
- Never script someone into a testimonial, and never present a sponsored
  appearance as a spontaneous opinion.


A NOTE ON THESE FORMS

These are plain-English templates written to be understood by the person
signing them, not to be clever. They have real legal weight. Before they
go into regular use they should be read by a lawyer, and Alpha Mart will
arrange that. Use them in the meantime — a signed plain form is far better
than no record at all.

Completed forms go back to Alpha Mart and are kept by Deustar Pty Ltd.
They are not shared with anyone and the contact details on them are never
used for marketing.
